- comments: observed no city of chicago certified food manager on the premises while tcs foods (sausages, chicken wings, pizzas) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels at the front prep area handwashing. Instructed management that all handwashing sinks must be equipped with a hand drying provision at all times; instructed to provide. Priority foundation 7-38-030(c). Citation issued. Paper towels are now provided at the handwashing sink.
40. Personal cleanliness
- comments: observed an employee preparing food without a hair restraint. Instructed management that all employees handling open foods must wear a hair restraint. Must provide.

- comments: observed expired food handler certificates. Instructed to provide non-expired and valid food handler certificates for all current food handlers and maintain.

- comments: previous core violations from 4/7/2022 report #2553997 not corrected: violation #47 (4-501. 11) observed torn gasket at pizza prep cooler (right side) where dough is being stored. Must repair. Violation #49 (4-602. 13) instructed to clean grease inside of cooking fryers and maintain. Violation #53 (5-501. 17) observed no lid on the trash can in the unisex bathroom. Must provide. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
