- comments: observed no hand soap provided at the exposed hand washing sink in the bar area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed approx. 40 live small flying insects at the bar, food prep area and basement storage area of the restaurant. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: clean the prep table in the basement and containers of soy sauce.

- comments: seal the wall and corner next to the hot water heater in the food prep area.
