1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic has no valid city sanitation certificate as required.

2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago food sanitation certificate posted or a city certified manager on duty while tcs foods are being prepared, handled and served such as pizza's and salads. Instructed manager that a valid city certified manager must be on duty/site at all times. Also must posted said city sanitation certificate. Prioity foundation citation issued #7-38-012.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the hand sink in the rear prep / dishwashing area in poor repair turn off) due to clog sink. Instructed manager to repair and maintain. Priority foundation citation issued 7-38-030(c).

38. Insects, rodents, & animals not present
- comments: observed 35 small live scattered through out premises on racks, walls, boxes and on ceiling tiles. Must remove and detail clean affected areas. Also call pest control for service. Priority foundation citation issued #7-38-020(a).

- comments: observed employees without any hair restraint in the food prepare. Must provide and maintain

- comments: must clean the interior of all the coolers and freezer to remove spills and debris.

- comments: must detal clean the washroom fixtures in both men & women washrooms.

- comments: observed cardboard on the floor in the walk in cooler. Must remove and detail clean.

- comments: observed the filter above the cooking equipment dirty. Must clean and maintain.

- comments: instructed manager that all city certified manager must have allergen training or citation will follow.
