3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towel in front and rear prep hand-sink. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory for raw or undercooked foods. Instructed to identify such foods by asterisking them to a foot note that states: ""items can be served raw or undercooked or contain undercooked ingredients"" and add a statement that reads: "consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions. " instructed to provide consumer advisory on all menus (print and electronic). (refer to cdph consumer advisory guide on cdph website) priority foundation 7-38-005

- comments: observed open boric acid spilled throughout prep areas in front and rear food prep area (where sushi is being prepared). Observed boric acid on food prep table, storage shelves. Instructed to remove, clean, sanitize and maintain said area. Priority violation. 7-38-015 citation issued.

- comments: observed pest activity with approx. 15 small flies noted during inspection. Small fly activity was noted in toilet room and water heater closet. Noted 3 live roaches seen in reach prep instructed to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

- comments: observed 3 dead roaches in cabients over 3 comp sink, under 3 comp sink and inside cold holding unit, instructed to clean and sanitaize and maintain

All retail food establishments shall contract with a licensed illinois exterminator who shall provide insect and vermin services at the establishment. Missing record of visits, missing copy of service ticket that sets forth pest control idph license number. Must provide and maintain all required pest control records and documents. Priority foundation violation. 7-38. 020 (c) citation issued instructed to provide.

- comments: observed rusted and food encrusted storage racks in all reach in coolers and refergerator and freezer units in, surfaces must be smooth and easily cleanable. Instructed to maintain

- comments: observed grease buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to degrease, clean and sanitize.

- comments: observed 3 compartment sink leaking and hand sink in toilet room areas leaking exceesivily from pipes, also noted pipes surround toilet also leaking, grease trap rusted and lid is breaking into pieces. Instructed to repair and maintain

- comments: observed plumbing in toilet room encrusted with grease and other slimelike substance leaking onto floor, instructed to repair and clean, sanitize and maintain

- comments: observed no commercial dumpster on site, (1006 retail food using residential dumpsters) pic indicated landlord told he can use residential dumpsters. Instructed to provide a contract with a licensed waste management operator and dumpster to be onsite. Noted facility uses non-commercial grade grease container without proper cover and not rodent proof. Must provide proper commercial grease container. Priority foundation violation #7-38-020(b). Citation issued.

- comments: observed grease buildup, food debris, and dirt on floor under cooking equipment, along walls and in all corners in prep, dish washing, storage. Instructed manager to clean floors under all equipment, along walls and in all corners in all areas. Observed dust buildup on mechanical fan, light shields in cooking area, dining area and ceiling and wall vents in prep and storage areas. Instructed manager to clean and maintain all fans, light shields and ceiling/wall vents.

- comments: observed wall base peeling off and missing throughout prep, dish, and storage areas. Instructed to repair and or replace and maintain
