- comments: observed no city of chicago sanitation certificate on site while tcs foods (rice, chicken, pork) are being prepared and served. Instructed person in charge to obtain city of chicago sanitation certificate and maintain on site at all times. Priority foundation #7-38-012. Citation issued.

- comments: observed no handwashing signage at front handsink. Instructed person in charge to provide and maintain handwashing sign at all times.

- comments: observed cardboard and foil on shelving and tabletops. Instructed person in charge to remove cardboard and foil and maintain areas smooth and easily cleanable.
