- comments: observed facility not meeting thawing requirements. Food handler thawing frozen chicken in two-compartment prep sink sitting in stagnant water. Informed the person in charge of proper thawing methods ( must utilize running water with enough volatility when thawing). Must meet proper thawing requirements and maintain

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed aluminum foil and cardboard being used as liners on prep tables and shelving units throughout the prep areas. Instructed the person in charge to remove aluminum foil and cardboard from shelves and prep tables and ensure all nonfood-contact surfaces remain smooth and easily cleanable.

51. Plumbing installed; proper backflow devices
- comments: observed slow draining exposed handsink near the compartment sink in the rear prep area. Instructed the person in charge to repair so that handsink drains properly and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris on floors throughout prep area and in the walk-in freezer. Instructed the person in charge to detail clean floors and maintain.

57. All food employees have food handler training
- comments: observed multiple food handling employees without food handler training certificates at inspection time. Instructed the person in charge to ensure all food handling employees obtain food handler training to comply with idph food handler requirements.

58. Allergen training as required
- comments: observed one certified food service manager without allergen awareness training certificate at the time of inspection. Instructed the person in charge to ensure that all individuals that have a city of chicago food service manager certificate obtain allergen awareness training and receive allergen awareness certificate.
