- comments: observed a missing handle on the three door reach in cooler in the rear of the facility dish washing area. Instrsucted to repair and maintain.

- comments: observed cutting boards in food prep area badly stained and deep cutting grooves. Instructed to replace all and maintain a every two hour cleaning and sanitizing unti they are replaced.

- comments: observed no covered receptacle in employee toilet room. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed missing ceiling tiles in the front food prep area and in the rear dish washing area. Instructed to replace and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: observed very low lighting in the rear near the employee toilet room. Instructed to provide brighter lighting.
