- comments: observed no city of chicago food sanitation manager on site while tcs foods are being prepped (such as chicken). Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Instructed must post a valid city of chicago food sanitation manager certification in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed incomplete vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005.

- comments: observed no hand soap available at handsink in prep/diswashing area. Instructed to provide handsoap at all times. Priority foundation violation 7-38-030 (c) citation issued

- comments: observed flour out of the original containers without labels containing the product name. Instructed to label all foods and spices with common name that are out of the original containers.

- comments: observed a missing handle on the three door reach in cooler in the rear of the facility dish washing area. Instrsucted to repair and maintain.

- comments: observed cutting boards in food prep area badly stained and deep cutting grooves. Instructed to replace all and maintain a every two hour cleaning and sanitizing unti they are replaced.

- comments: observed the gasket seals on the opening of the three door cooling unit in the rear broken and damaged. Instructed to repair and maintain.

- comments: observed no chlorine or quats sanitizer testing devices on site at time of inspection. Instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: observed no covered receptacle in employee toilet room. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed floors throughout facility with a build up of dirt, grease and small debris under and round all cooking and non cooking equipment. Observed grease build up in rear near grease filtering machine. Observed food splatter and stains on walls in rear dish washing area. Instructed to clean and maintain all areas.

- comments: must detail clean the filter and hood above the cooking equipment to remove grease and debris.

- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.

- comments: observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.
