1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic with no valid city of chicago certified manager certificate.

2. City of chicago food service sanitation certificate
- comments: pic with no valid city of chicago certified foodservice manager certificate while tcs foods are being prepared(chicken, fish). Instructed manager that valid city of chicago certificate must be on site at all times for review. Priority foundation 7-38-012(citation issued).

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy available. Instructed manager to provide documentation of employee responsibility to report any illness transmissible through food. Priority foundation 7-38-010 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit or diarrhea clean up kit on premises as required; must provide. Instructed the person in charge that a vomit and diarrhea clean up procedure must be documented and the necessary supplies on hand in the event of a vomit and diarrheal incident on premises. Priority foundation violation 7-38-005 citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels or hand drying provision at handwashing sinks in prep area. Instructed person in charge to provide and maintain at all times. Priority foundation. 7-38-030(c). Citation issued

- comments: observed no proper thermometer on site for taking food temperatures. Instructed person in charge to provide and maintain on site at all times. Priority foundation violation 7-38-005 citation issued.

- comments: interior of walk in cooler door in poor repair, exposed insulation. Instd to repair and maintain smooth, even, easily cleanable

- comments: all equipment through out including all basins of sinks, storage shelves, self service beverage dispenser, interior bottom of fryers, prep tables, exterior/interior of all coolers including door gaskets not clean, excessive food debris. Instd to clean all and maintain

- comments: no hot running water under city presssure as required, handwash sinks 84. 0f, 3-compartment sink at 86. 0f. Instd to provide sufficient hot running water under city pressure at all times (100f-120f at handwash sinks, 110f minimim at 3-compartment sink). Citation issued 7-38-030(c) priority

55. Physical facilities installed, maintained & clean
- comments: odor noted in rear near walk in cooler. Instd to remove odor and maintain. Floor, floor drains, walls and wallbase through out not clean, heavy accumulation of dirt and debris. Instd to detail clean and maintain.
