- comments: no original copy of the city of chicago food service managers certificate as required; instructed to provide and maintain at all times. Priority foundation violation 7-38-012 citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no completed employee health policies on premises as required; instructed to provide and maintain. Priority foundation violation 7-38-010 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: no vomit and diarrhea clean up kit on premises as required; instructed to provide and maintain at all times. Priority foundation violation 7-38-005 citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no handwashing sink found in rear dishwashing area as required. Sink was located in the utility room behind the three comp sink and was removed. Establishment was licensed in february of 2024. Instructed to replace handwash sink in dishwashing area. Priority foundation violation 7-38-030(c), citation issued.

- comments: found cooked rice on steam table at 86f. Instructed to hold rice at 135f or above at all times. Priority violation 7-38-005 citation issued. Discarded nine lbs of cooked rice valued at $40.
