- comments: observed no hand washing signs at hand washing sinks in washrooms. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

13. Food in good condition, safe, & unadulterated
- comments: observed bags of chips and popcorn gnawed and adulterated with mice droppings and displayed for sale on shelving units in sales area. Instructed manager to discard. Food packages shall be in good condition and protect integrity of contents so food is not exposed to adulteration or potential contaminants. Priority foundation 7-38-005 citation issued.

38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor and cardboard on chip display shelves in sales area, on floor near two-compartment sink in liquor storage area, under bubble gum machine and unused equipment, along walls and corners in rear storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

49. Non-food/food contact surfaces clean
- comments: observed dust, dirt and debris on display shelves in front counter and sales area. Instructed manager to clean and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in women and unisex washrooms. Instructed manager to provide.

55. Physical facilities installed, maintained & clean
- comments: observed stained ceiling tiles behind front counter, openings around pipe fittings near two-compartment sink, openings in walls in rear storage areas. Instructed manager to seal all openings in walls and ceilings, along baseboards, around pipe fittings in all areas.

56. Adequate ventilation & lighting; designated areas used
- comments: observed no operable mechanical ventilation in east women washroom. Instructed manager to provide.

60. Previous core violation corrected
- comments: previous core violation not corrected from report #2385570 on 7-30-20:55- observed the wall base in poor repair behind front liquor display shelving(missing wall base), also noted floors inside the walk in cooler in poor repair(broken tiles) must repair and maintain. Priority foundation 7-42-090 citation issued
