- comments: 6-501.111 observed 3 live large cockroach crawling on floor in front prep under prep table containing hot hold units, on the wall in rear prep area near cooling unit and dishwashing area during time of inspection. Recommended contacting pest control for service and cleaning and sanitizing the affected area. Must eliminate pest activity on site. Priority foundation violation. 7-38-020 (a). Citation issued.

- comments: 4-201.11 observed rubber gaskets on cooling units throughout prep area in disrepair. Instructed to replace and maintain in good condition.
