- comments: observed no person in charge with sanitation manager training.

- comments: observed no valid sanitation certificate on site at time of inspection. Valid certificate and manager must be on site at all times tcs foods (chicken, shrimp) are being prepared and served. Priority foundation #7-38-012. Citation issued.

- comments: observed rear exterior door with 1/2in gap around bottom and side. Instructed person in charge to repair door to be tight fitting.

- comments: observed approximately 75 mouse droppings scattered throughout facility in the following areas; around grease trap at three compartment sink, in rear area under shelving and between chest freezers. Instructed person in charge to remove all mouse droppings, sanitize areas and inform pest control of activity. Priority foundation #7-38-020(a) citation issued.

- comments: observed build up of food debris on bottom of multi door reach in cooler in rear prep area. Instructed person in charge to clean and maintain.

- comments: observed pipe leaking at utility sink in prep area. Instructed person in charge to repair and maintain.

- comments: observed no covered receptacle in restroom. Instructed person in charge to provide and maintain covered receptacle for disposal of sanitary products.
57. All food employees have food handler training
- comments: observed no food handler certificates on site for food handler employees. Instructed person in charge to direct food handler employees to obtain training and maintain certificates on site.
