Report date: Dec 6, 202210. Adequate handwashing sinks properly supplied and accessible
- comments: observed the facility missing the employee handwashing signage at the handwashing sinks in the front package/order prep area in the right side of the restaurant/ dining area and in the dishwashing area. Informed person in charge to obtain and maintain signage.
38. Insects, rodents, & animals not present
- comments: observed the pest control logbook on site with service reports from only 2019 and is not up to date with the required information: record of visits from this year (2022), missing copy of service ticket that sets forth pest control idph license number. Must provide and maintain all required pest control records and documents from the current year. Priority foundation violation 7-38. 020 (c), citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed the handwashing sink in the dishwashing area near the last compartment of the three-comparment sink without a splash guard. Instructed to provide and maintain a smooth, and easily cleanable splash guard at the said handwashing sink.

- comments: observed the ice scoop stored on top of the ice machine in the right side of the dining area of the facility. Instructed to properly store the said ice scoop and maintain.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed the tableware (forks, spoons, and butter knives) not stored properly in the package/order prep area on the right side of the dining area in the facility. Instructed to invert said tableware and maintain.
49. Non-food/food contact surfaces clean
- comments: observed the inside of the deep-frying unit, in the food prep area with severe grease build up on the pipes and conduits. Instructed to clean, sanitize and maintain said equipment.
55. Physical facilities installed, maintained & clean
- comments: observed the ceiling in the rear food prep area with slight dust build up on the ventilation attachment and the ceiling tile. Instructed to remove dust, clean, sanitize, and maintain said ceiling tile and the ventilation attachment.
56. Adequate ventilation & lighting; designated areas used
- comments: observed the ventilation system with grease build up above the cooking equipment. Instructed to clean and maintain said ventilation system more frequently and maintain
57. All food employees have food handler training
- comments: observed no food handler certificates on site. Instructed to provide and maintain records of the food handlers certificates on site.
58. Allergen training as required
- comments: observed the certified food manager on duty with an expired allergen training certificate on site at this time. Instructed the certified food manager to retake the allergen training course and maintain records on site at all times.
