- comments: observed no written employee health policy on the premises. Instructed management to provide a signed employee health policy for each employee and maintain as required. Priority foundation violation 7-38-010,citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: instructed to provide adequate, designated handwashing sinks properly equipped with soap, hand drying and required signage at the following areas:1st floor bar and roof top bar also, roof top bar needs a separate dump sink. Priority foundation violation 7-38-030(c),no citation issued.
38. Insects, rodents, & animals not present
- comments: observed rear exit door the leads to alley with gap opening approx:1/4" at bottom left hand corner. Instructed to make door tight fitting all across the bottom to prevent pest entry and maintain.

- comments: observed evidence of pest activity on the premises. Approximately 40 live small flying insects observed on the walls and ceilings and equipments in the rear pastry prep/dishwashing area and approximately 30 rat droppings on the floor underneath storage racks in the rear basement dry storage areas. Additional pest control service is needed to eliminate the pest activity and maintain. Priority foundation violation 7-38-020(a),citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed a torn door gasket inside of the 2 door freezer located at rear pastry prep area. Instructed to replace the torn door gasket and maintain.
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize the rear basement dry storage areas and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6" inches to facilitate cleaning and maintain.
58. Allergen training as required
- comments: observed the certified food manager without the allergen training certificate. Allergen training is required for all employees that have a city of chicago food sanitation certificate. Instructed to complete training and keep certificate on site.
