    - comments: found a food employee drinking coffee from a cup without a lid and eating a pastry in the rear prep area, and storing the beverage and food on top of a prep cooler. Instructed management that employees must consume beverages/food in designated areas to prevent contamination of food, equipment, utensils, etc.

    - comments: observed an incomplete consumer advisory for raw/undercooked foods - the advisory is missing a disclosure statement. Instructed management to add a disclosure statement to the current advisory that says 'items can be served raw or undercooked or contain undercooked ingredients'. Priority foundation 7-38-005.

    35. Approved thawing methods used
    - comments: found a package of frozen shrimp submerged in a container of water, thawing at room temperature. Instructed management tcs foods must be thawed according to food code, such as under refrigeration that maintains 41f or below or under running water (70f or below).

    - comments: found clean utensils stored hanging on the wall directly next to the mop sink. Instructed management to relocate clean utensils to where they are not exposed to splash/potential contamination from the mop sink.

    - comments: unable to locate and verify food handler certificates during inspection. Must provide and keep certificates on site.

    - comments: previous core violation from report #2290954 dated 6/5/2019 not corrected: 'observed the certified food manager without an allergen training certificate.     On Tueday the 4th January 2022 I bought Tesco Ready Roll Puff Pastry(375g),use by 24 January 2022, along with other groceries. On Wednesday (5th) my wife made a Mince Steak pie with all fresh produce and used half of Ready Roll Puff Pastry (the other half was... See More put in freezer, same day). I had half the pie for my tea on Wednesday (5th), during early hours of Thursday (6th), I felt uncomfortable with stomach cramps and nausea, at 07.40am I projectile vomited, followed immediately by diarrhea. During the rest of that day, I repeatedly had stomach cramps and diarrhea. That night (6th) and early hours of Friday (7th) was also uncomfortable and again at about 8.30am Friday (7th) I again projectlie vomited, followed immediately by diarrhea. During the rest of that day, I repeatedly had stomach cramps and diarrhea. My wife went to our local Pharmacy and described my symptoms and they recommended Dioralyte relief (6 sachets). I took 4 sachets (@ 4 hour intervals) that day which helped relieve the stomach cramp pain and took the other two sachets on Saturday (8th). At this point, I hadn't eaten for 3 days (only drank water). Sunday (9th) felt a little better and with my stomach protesting at no food, I had two pieces of dry toast for breakfast and later Tomato Soup for tea (no sickness or diarrhea that day), however early the following morning Monday (10th) at about 8am I felt sick and vomited up (what looked like the tomato soup). After that I was ok and no more symptoms. On Tuesday (11th January 2022) I saw the product recall on my computer, (which relieved my wife, who was worried that she had poisoned me).     The FSAI announced the recall of specific batches of various raw pastry and dough products by BakeAway due to the presence of Salmonella. The recalled products have been distributed nationwide in Ireland.
