- comments: observed hand washing facilities not accessible in front of the retail prep/ serving area. Observed ice inside of said hand sink, boxes covering hand sink are not able to wash hands. Articles were removed from the sink during inspection. Must keep accessible and maintained at all times. (priority foundation 7-38-030(c)(citation issued)(cos)

- comments: must label food storage containers when food is not in original package. (squeeze bottles,bulk,etc)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 75 or more live small flies on cabinets, walls, ceiling, exterior of walk-in cooler, and flying around in breakroom, prep areas, dish washing areas, and packaging. Also evidence of appx. 20 or more live large flies flying around prep and dish washing areas. Recommended to have a pest control operator to service premises on or after 8/18/25. - comments: observed water leaking from overhead sill of roll up door in packaging area must repair and maintain.

- comments: must invert multi-use utensils on dish storage shelves in dish washing area.

- comments: must eliminate crates used for food storage in rear storage and pastry walk in cooler and provide adequate shelving units.

- comments: must clean food debris build up from 2- door reach in cooler next to ice machine,interior panel of ice machine,window shades in dish washing area.

- comments: must repair or replace leaky pipe at 3- compartment sink and remove tape from same to be in good repair and leaky air conditioner unit in sandwich prep area leaking into a bucket of standing water.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage area dumpsters overflowing with trash bags, lids wide open, food splattered on ground around dumpsters. Must clean grounds around dumpster, close lids to be tight-fitting, and maintain. (priority foundation 7-38-020(b)(citation issued)
