11. Food obtained from approved source
- comments: observed food from an unapproved source being sold in the establishment. Observed desserts (flan and cheesecake) being sold in a cooler on the premises from alisha's sweet treats and things. According to management, the food is being produced from a food establishment which does not have a shared kitchen license and alisha does not have a shared kitchen user license. Management instructed that all food must be prepared in a properly licensed food establishment. Priority violation 7-38-005. Citation issued. - cos

45. Single-use/single-service articles: properly stored & used
- comments: observed single use plastic forks, knives and spoons stored improperly on top of the counter top for customers. Management instructed to store single use utensils with the handles up so that they are not contaminated.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed a rusted surface on the bottom shelf of the prep table stored in the rear food prep area. Management instructed to paint over the rust or replace the table.

49. Non-food/food contact surfaces clean
- comments: clean the prep coolers, prep tables, interior of the 3 door freezer #6-8, and stainless steel dry storage shelves above the prep table.

51. Plumbing installed; proper backflow devices
- comments: repair the drain line water leak at the middle compartment of the 3 compartment sink.

57. All food employees have food handler training
- comments: observed several food handling employees without a food handler certificate. Management instructed that all food handling employees must show proof of training.
