- comments: no valid city of chicago food sanitation manager certification provided nor posted while preparing tcs foods from raw ingredients such as shrimp and fish. Instructed manager to provide. Priority foundation violation. Citation issued. 7-38-012.

- comments: no handwashing signage provided at the handwasing sinks located in the back prep area. Instructed manager to provide.

- comments: observed container of flour on the shelf in the dry storage area uncovered. Instructed to keep flour covered when not in use to prevent from contamination.

- comments: observed wet wiping cloths in the front and back prep area and dishwashing area. Instructed to keep wet wiping cloths in proper sanitizing solution when not in use.
