- comments: observed 10 mice droppings scattered on the floor in the rear storage room behind shelving, noted 30 mice droppings under 1 comp sink on the interior ledge, and 5 droppings on the floor next to the 1 comp sink in the dish washing area. Priority foundation 7-38-020(a) citation issued.

- comments: observed a leak at the base/neck of the faucet at the front 3 comp sink. Must repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: floors need cleaning in the following areas: walk in cooler in corners, dry storage room under shelving along the wall base and where droppings were noted. Also detail clean all floor drains to prevent possible breeding place for pests.
