- comments: consumer advisory noted on the menu is incorrect in disclosing items that are not raw, undercooked or that may be cooked to order such as all chicken dishes. Reviewed consumer advisory requirements, given sample memo and instructed to correct mislabeled items on the menu. Priority foundation violation.

- comments: in use, wet wiping cloths noted must be held in the container of sanitizing solution provided between use.

- comments: all stored items under the front prep/server counter and the rear prep area plastic storage bins must be elevated from the floors. Floors must be accessible for cleaning and monitoring.

- comments: i noted a clean rubber spatula in poor repair with torn/missing rubber. Instructed to replace.
