10. Adequate handwashing sinks properly supplied and accessible
- comments: we observed no soap provided at front and rear handwashing sink in both prep areas and dishroom,also hand washing sink at bar area. Soap must be provided at said sinks for proper handwashing. Upon my request soap was provided by the employee. Priority foundation violation: 7-38-030(c),citation issued.

- comments: no paper towel provided at front handwashing sink in prep area. After my request employee provided. It at said sink. Priority foundation violation:7-38-030(c). Consolidated with above citation.

- comments: handwash sink at bar area not accessible for hand washing inside the bowl empty soap dispenserand a glass with creamy liquid inside. In front and site plastic containers filled with items (glasses and other kitchen utensils) buckets ets. Must remove items mentioned. Area presently in use. Priority foundation violation:7-38-030(c). Consolidated with above citation.

22. Proper cold holding temperatures
- comments: we observed tcs food stored inside the reach-in cooler at improper temperature: 6lb cooked rice with carrots and corn in sauce at temp of 49. 8f to 50. 5f(food was cooked the the before);6lb of white cooked rice with carrots and corn at temp of 49. 2f to 53f. Garlic in oil(1/2 pound) at temp 48f( food was cooked two days ago,according from label provided with products) ; empanadas refilled with shrimp at temp of 49. 7f to 51f. Cooked mussels in half shell at temp of 48. 7f;7 each of one quart of half & half at temp of 50. 1f to 51f. Cold food must maintain temp of 41f and below. Food mentioned was discarded and denatured. Pounds 25,value 200. Priority violation:7-38-005. Citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: consumer advisory posted on site, however no reminder (in the menu), and disclosure (by the food items) for all undercooked foods(steaks,fish etc). Must indicate on menu "cook to order" phrase for eggs, steaks, fish etc. Priority foundation violation#: 7-38-005. Citation issued.

- comments: label food in containers with common name at storage area

39. Contamination prevented during food preparation, storage & display
- comments: splash guard is needed on both side of handwashing sink due to space available is used to store food and food containers. Surface must be smooth,cleanable and non-absorbent surface.

51. Plumbing installed; proper backflow devices
- comments: mechanical drain at three compartment sink broken. Repair issue.

60. Previous core violation corrected
- comments: previous core violations not corrected from 12-5-2024,report# 2608507 47)4-101. 11(b, c, d, e): must not use plastic grocery bags as food containers inside the chest freezers. Must not use cardboard boxes as food containers. Must use washable containers or food grade disposable plastic bags. #49) 4-602. 13: accumulated grease and food debris on walls, utility connections and equipment surfaces behind and inside cooking equipments. Must clean and maintain. #51) 5-205. 15: repair leaking hot water handle at three compartment sink at bar area,also repair leaking pipe under handwashing sink at same area. #55 )6-201. 12: grease build-up on floors under cooking equipments,also debris on floor under shelves and along baseboards in storage area. Clean and maintain #57) 2-102. 13: additional food handler training certificate is required on site,instructed to provide priority foundation violation:7-42-090,citation issued
