- comments: observed black and pink substance build up on the interior panel above the ice on the inside of the ice machine for the self-service beverage dispenser in the dining area. Ice used for human consumption. Instructed must remove build up, clean, and sanitize with appropriate chemicals to avoid contaminating ice. Priority foundation 7-38-005, citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed the floors under the three orange, carbon dioxide tanks near the rear mop sink with black residue and not maintained. Also, observed slight food debris on the floor in the walk-in cooler. Instructed to deep clean and maintain mentioned floors.
