- comments: observed water actively leaking from ceilings in front preparation area and rear hallway with plastic bins and bowls placed on floor and on & in between equipment catching water. Manager states the roof needs repairs and will be addressed soon. Instructed to take measures to prevent cross contamination,clean and sanitize all surfaces and equipment. - comments: must clean debris build up from walk in cooler shelving units. Must clean water from exterior of equipment that is leaking from ceilings throughout.

- comments: must clean standing and pooling water from floors in hallway and rear storage room and front prep area and keep dry.
