Report date: Apr 2, 202539. Contamination prevented during food preparation, storage & display
- comments: observed water actively leaking from ceilings in front preparation area and rear hallway with plastic bins and bowls placed on floor and on & in between equipment catching water. Manager states the roof needs repairs and will be addressed soon. Instructed to take measures to prevent cross contamination,clean and sanitize all surfaces and equipment. (priority foundation 7-38-005)(citation issued)

- comments: must clean debris build up from walk in cooler shelving units. Must clean water from exterior of equipment that is leaking from ceilings throughout.
55. Physical facilities installed, maintained & clean
- comments: must clean standing and pooling water from floors in hallway and rear storage room and front prep area and keep dry.
