16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11(b-e) observed a cutting board with dark grooves. Instructed to remove dark grooves or replace.

22. Proper cold holding temperatures
- comments: 3-501. 16(a:2,&b) observed a container of raw chicken tenders at an inadequate cold holding temperature of 57f in a refrigerator during the inspection. Instructed to discard and denature said tcs foods and always maintain all tcs foods at 41f or below at all times. Manager discarded said tcs foods during the inspection. Priority violation. Citation issued 7-38-005.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed one live cockroach crawling from under the prep cooler to under the cooking equipmet during the inspection. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed excessive grease and food debris in between cooking equipment and on the electrical piping behind cooking equipment, observed accumulated dust debris on the condenser of the prep cooler and accumulated food debris on all prep cooler and refrigeration gaskets, doors and door handles, and inside of the food refrigeration units. Instructed to clean and sanitize all.

55. Physical facilities installed, maintained & clean
- comments: 6-201. 11 observed uneven surface in various areas of the facility, floor under refrigeration unit and in the toilet room wall. Instructed to make all surfaces smooth and easily cleanable.
