- comments: observed unapproved/unauthorized toxic white substance noted on storage shelves.

- comments: interior cabinets of fryers with excessive grease accumulation, legs/wheels of all equipment, storage shelves, door gaskets and tracks of all coolers not clean, debris. Instd to clean and mainatin same.
55. Physical facilities installed, maintained & clean
- comments: floor along wallbase through premises, and under fryers with heavy grease, debris accumulation. Instd to clean and maintain same. Walls through out premises not clean, roache feces.
59. Previous priority foundation violation corrected
- comments: live multi generational roaches observed on premises, over 10 baby roaches crawling underneath front service counter, 3 large roaches crawling on floor behind hot holding cabinet at front prep area, 1 large roach underneath menu board at front service counter, 1 large roach in hole at exterior side of service counter, 1 large roach crawling on chemical bottle in rear near exit door. Roach feces noted on walls through out premises around wire. Instd to remove all roaches, roach activity/feces, clean and sanitize all affected areas and have updated pest control service receipt.
