- comments: found various kind of tcs foods at improper temperature stored inside the serving cooler at butcher's area: ceviche at temp of 48. 7f; ham at temp 47f; raw chicken legs at temp of 48. 6f; fresh cheese at temp of 45f 47f food discarded and denature. Pounds 115,value:371 cold food must maintain temp of 41f and below. Priority violation:7-38-005,citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed the establishment serving ceviche(raw food) with no consumer advisory, disclosure or reminders to the customers. Instructed to have a consumer advisory on the menu with a disclosure notice and reminder statement. Priority foundation violation 7-38-005

37. Food properly labeled; original container
- comments: food products stored in container on top the display cooler at butcher's area must be labeled with common name

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: condenser unit inside the walk-in cooler at butcher's area pipe is leaking. Also condenser unit at display cooler at same area with ice build-up(south end of the unit)instructed to repair.

55. Physical facilities installed, maintained & clean
- comments: broken ceiling and walls inside the liquor room must be smooth and easily cleanable,in basement area.
