10. Adequate handwashing sinks properly supplied and accessible
- comments: observed exposed hand washing sink in rear prep area clogged and water not draining so hands can not be washed there. Instructed to repair and maintain all exposed hand washing sinks. Priority foundation violation 7-38-030(c). Citation issued.

22. Proper cold holding temperatures
- comments: observed tcs food at improper temperature requirement stored sitting outside of walking in cooler. Found temperature of bbq chicken at 78. 8f instructed all tcs foods must maintain cold holding temperature of below 41. 0f and hot holding temperature of above 135. 0f. Operator voluntarily discarded and denatured foods during inspection. Priority violationi 7-38-005. Citation issued.

23. Proper date marking and disposition
- comments: observed prepared tcs food items cooked chicken, cooked pork, bbq chicken, etc. ) in walk in cooler without labels. Instructed to provide discard date labels (not to exceed 7 days) on all foods prepared on site and held for more than 24 hours and maintain. Priority foundation violation 7-38-005. Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in walk-in cooler and freezer. Instructed manager to elevate all food six inches off floor.

41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted some use of milk crates for elevation at the walk-in cooler and dry storage area. Instructed not to use milk crates for elevation to prevent hiding places for pest. Must provide appropriate shelving for storage. All food items must also be stored 6' away from the wall

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cutting board on butcher line covered with deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: found the three compartment sink not maintained; sink is slow draining. When water from one compartment is drained; water backs up into adjacent compartments. Instructed management to get drainage system serviced. Priority foundation 7-38-025. Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed garbage can inside unisex toilet room without covering inside of toilet room. Must provide lid and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed debris around dumpster. Instructed to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed rusty and uneven floor inside of walk in freezer. Must make smooth and easily cleanable. Must repair or replace damaged wall tile under hand sink in rear prep/dish washing area,damaged and missing floor tiles on sales floor.

- comments: observed excessive stored items (food items inside stacked crates, cooking equipment, store materials and other stored household items throughout. Must remove all unneccsary items in order to prevent pest harborage and maintain premise.

57. All food employees have food handler training
- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain records thereof on site at all times.

58. Allergen training as required
- comments: observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.
