- comments: observed facility without a valid city of chicago food service sanitation manager certificate posted, at the time of inspection, while time/temperature control for safety, tcs, foods are stored, prepared and/or served. Instructed to provide. Priority foundation violation. 7-38-012. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed facility with a bodily fluid spill kit without adequate supplies (disposable mop, disposable latex gloves, disposable mask, and adequate sanitizer *must eliminate norovirus*) on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Instructed to provide a complete kit with adequate supplies for vomit and diarrhea clean up. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed no ambient air thermometers inside the reach of display coolers in the front area of facility near checkout. Instructed to provide.

- comments: observed food stored in containers in walk-in coolers without proper labeling of name. Instructed to provide.
39. Contamination prevented during food preparation, storage & display
- comments: observed improper storage of food containers stored on the floor in the walk-in cooler and storage areas. Instructed to store properly store by elevating at least six inches from the floor on raised shelving units.

- comments: observed improper storage of in use utensils on cutting board in prep area in rear deli area. Instructed to store properly between usage.

- comments: observed cardboard used as liner on shelving in walk-in cooler. Instructed to remove and provide smooth cleanable surface.

- comments: observed milk crates being used as shelving in prep and storage areas. Instructed to remove and provide shelving that is 6" from the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed cutting boards in the rear prep area at the deli with open seams and dark stains/grooves. Instructed to refinish/replace.

- comments: observed outside dumpster with lids not closed due to trash overflowing. Instructed to correct to ensure tight fitting lids. Priority foundation violation. 7-38-020(b). Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed tape on cracked/broken sliding door to refrigerator/cold-holdin unit in the rear deli area and observed broken faucet at the 2-compartment sink in the rear of facility. Instructed to replace/repair.

- comments: observed no food employee with proof of allergen training. Instructed to provide for city of chicago food service sanitation managers.
