2. City of chicago food service sanitation certificate
- comments: observed no certified manager on duty from 1:50pm-2:15pm while tcs items like sushi were prepared. Certified manager arrived on site at 2:15pm. Instructed to maintain an employee holding a city of chciago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit on site for responding to vomit and diarrhea. Instructed to maintain a kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observe no handwashing signage at exposed handwashing sink located at sushi prep area. Instructed to provide.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed crack in plastic door for fish cooler located on top of sushi bar and low battery at soap dispsener located at sushi prep station. Instructed to repair or replace and maintain in good condition.

- comments: observed 2 cutting boards in sushi prep area covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces.

- comments: observed low water pressure at exposed handwashing sink located in rear dish dish area. Instructed to repair or replace and maintain in good condition.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed lid is on used cooking oil container is broken. Instructed to repair or replace and maintain in good condition.

56. Adequate ventilation & lighting; designated areas used
- comments: observed slight build up of grease on hood above main cook line and build up of dust on vent above utility sink. Instructed to clean and maintain in good condition.
