- comments: observed self-serve ice machine located in dining area with a build-up of black and brown slime substance, dripping onto ice cubes stored below the ice bin. Ice is used for human consumption. Also, black slime at the exterior of the unit between the ice bin and the top part of the unit. Interior plate of ice bin with excess rust; external black cover not clean. Also noted green, black, and brown substance on the exterior of the ice machine in the kitchen. Instructed to clean, sanitize, and maintain. Priority foundation violation #7-38-005. Citation issued
58. Allergen training as required
- comments: noted no proof of allergen training for all certified food managers. Instructed any persons with the city of chicago food sanitation certificate to comply with the completion of training and maintain documentation on site at all times.
