- comments: observed bins/boxes of food stored on the floor in the walk in cooler at time of inspection. Instructed manager to elevate all items off the floor at least 6 inches. Manager elevated all items at time of inspection.

- comments: observed the bottom shelves of the standing freezer in the rear prep area to be heavily soiled with loose food debris and some spillage. Instructed manager to clean and maintain interior shelves free of excessive food debris build up.
55. Physical facilities installed, maintained & clean
- comments: observed mops stored on the floor and in mop buckets near the mop sink at time of inspection. Instructed manager to store all mops appropriately to allow to air dry in between use.

- comments: observed the paper towel dispenser at the rear hand washing sink near dish machine to be not functional at time of inspection. Instructed manager to repair/replace the not functional paper towel dispenser/holder.
