- comments: observed raw animal origin pork and chicken being improperly stored with fresh, ready to eat cilantro and prepared salsa. Cilantro observed stored directly on top of raw chicken and a package of raw pork stored next to, touching a loosely covered container of salsa all inside the rear tall reach-in cooler. All foods removed and rearranged. Instructed to properly store ready to eat foods protected. Priority violation 7-38-005. No citation issued.

23. Proper date marking and disposition
- comments: observed prepared tcs ready to eat cheese inside a cooler with expired use by date. Cheese prepared on 5-4-20 with a use by date of 5-7-20. Instructed to minimize large quantities of prepared foods and discard any expired foods. Priority foundation violation 7-38-005. No citation issued.

- comments: all bulk food and spice containers must be labeled with the products common name.

38. Insects, rodents, & animals not present
- comments: observed approximately 75 mice droppings behind and along the side of the box freezer located in the hallway. Instructed to remove all droppings, sanitize all affected areas and recommend having the pest control co. Service establishment for mice. Priority foundation violation 7-38-020(a). No citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: interior door of the front prep one door cooler in poor repair. Open holes with exposed insulation and repaired with duct tape. Must replace.

- comments: must not use milk crates as a means of elevation throughout dry storage hallway and kitchen. Shelving must leave floors accessible for cleaning and monitoring.

- comments: missing/uneven counter-top tiles at the front prep hand washing sink. Must replace to make smooth and cleanable. ----must not use cloth towels as liner for shelving under the microwave oven.

55. Physical facilities installed, maintained & clean
- comments: observed wadded paper as crack and crevice sealant in the washroom ceiling and wall. Must be smooth and cleanable.

58. Allergen training as required
- comments: no proof of allergen training or certificates for all the city of chicago certified food managers. Must provide for all.
