2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling�time and temperature control for safety(tcs) foods (tacos, chicken, beef) without original city of chicago food service manager and certificate on site. Informed person in charge of city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean-up policy and supplies to handle bodily fluid spills on site properly. Priority foundation violation #7-38-005. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand signage at the hand sink in the front and rear food prep area. Instructed to provide and maintain.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice scooper being stored on top of the mat behind the bar with the food contact surface exposed. Instructed on how to store ice scoop between uses properly.

- comments: observed grab & go packaged cheesecake slices in the display cooler on the front counter, not properly labeled and with missing allergen information on the labels. Instructed person in charge to properly label all packaged food items with identity, net weight, ingredients, list major allergens, business name and address where made and packaged. Priority foundation violation #7-38-005. Citation issued.

41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloths on several prep surface areas. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food. Must maintain

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed food storage containers and in-use cooking equipment/utensils (pots, pans, etc. ) too large for 3-compartment warewashing sink. Informed person in charge sink compartments shall be large enough to accommodate immersion of the largest equipment and utensils. If said equipment or utensils are too large for warewashing sink, a warewashing machine or alternative equipment must be provided or written procedure how dishes will be cleaned. Priority foundation violation #7-38-025. Citation issued

55. Physical facilities installed, maintained & clean
- comments: observed mop in a position that does not allow to dry properly and prevent pest harborage located in the ulitity sink. Must store mop after use in a mounted position that allows for air-drying without soiling and pest harboraging.

- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation.
