- comments: observed no hand washing sign at hand washing sink in men washroom. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed live roache crawling on the wall in the rear prep area and near the utility sink. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items not covered and stored on floor inside walk-in cooler. Instructed manager to cover and elevate all items to prevent contamination.
