- comments: observed no one on site with a city of chicago certifeid food manager certificate while time/temperature controlled foods including chicken and shellfish are served. Priority foundation violation 7-38-012

- comments: found no employee health policy on site. Instructed to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010
16. Food-contact surfaces: cleaned & sanitized
- comments: observed small low temperaure dish machine behind bar with 0ppm of sanitizer detected. Machine was not in use at time of inspection. Machine was tagged priority violation 7-38-005. No citation issued.
36. Thermometers provided & accurate
- comments: observed broken thermometer in reach in cooler on serving line. Replace and maintain.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no test strips on site for low temperature test strips on site. Must have appropriate test strips for low temp machine in kitchen, behind bar and approprite test strips to test sanitizer in 3 compartment sink. Priority foundation violation 7-38-005

- comments: observed no proof of food handler training for any of the employees who handle food. Must provide and maintain.

- comments: observed no proof of allergen training for certified food managers. Must provide.
