- comments: observed bulk containers in rear kitchen shelf without a label. Instructed to provide the common name on all bulk containers to properly identify item(salt, sugar, flour etc).
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premises. Observed 3 live cockroaches on the floor and wall and ceiling in the basement food prep/dishwashing area and approximately 25 rodent (mice) droppings on the floor and metal rails inside of the large baking oven in the main kitchen prep area also,approximately 15 small flying insects in the basement food prep/dishwashing area. Instructed to detail clean and sanitize and remove all evidence of pest activity from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed dead cockroaches on glueboards on the floor inside of the large baking oven and in the basement rear dry storage room. Instructed to remove and clean all dead cockroaches from the premises.

- comments: instructed to remove worned-out blackened caulk around the 3-compartment sink in the main kitchen prep area and maintain so that is smooth,easily cleanble surface and maintain.
49. Non-food/food contact surfaces clean
- comments: instructed to detail clean 1st floor/basement kitchen storage racks,interior of coolers,tops/bottoms of prep tables,interior of rolling baking utility carts, deep fryer and interior of cooking stove and interior of oven also,storage shelf behind the front service counter.
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize the basement rear storage room and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.
56. Adequate ventilation & lighting; designated areas used
- comments: instructed to move gas stove so that is directly underneath the ventilation hood system and maintain.
