Report date: Aug 7, 202410. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels provided at both handwashing sinks: front prep and dishroom. Also, no paper towels provided at staff washroom. Upon my request paper towels provide at said sinks. Priority foundation violation:7-38-030(c), citation issued.

- comments: handwashing sink at rear prep area not accessible for for handwashing due iside the said sink was stored dirty containers and multi use utensils. Instructed handwashing sink must be use only for handwashing. Manager corrected the problem. Priority foundation violation:7-38-030(c),consolidated with above violation.
37. Food properly labeled; original container
- comments: food removed from original container must be labeled,also dry food in plastic container must be labeled with common name
38. Insects, rodents, & animals not present
- comments: we observed small flies on site, approximately 15. Dining area, prep and dish room especially at the north wall under the 3 compartment sink. One live roach on wall above the three compartment sink. Remove pest clean and sanitize area. Priority foundation violation: 7-38-020(a), citation issued.
40. Personal cleanliness
- comments: upon my arrival not all employees had effective hair restraints in the prep area. Upon my request all employees provide hair restraints.
41. Wiping cloths: properly used & stored
- comments: observed dirty cloths stored on top prep tables and cutting board. Must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.

- comments: broken rubber gasket inside the two door reach-in coolers in prep area. Repair issue.

- comments: encrusted grimes with ice inside the reach-in freezer. Instructed to clean and maintain.

- comments: wall caulking along the three compartment sink in poor repair, cracked/damaged and black slimes substance must replace. Surface must be smooth cleanable and non-absorbent material
