- comments: observed opening at base of garage rear exit door. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed approximately 25 mouse droppings in facility. Observed mouse droppings on floor in teacher resource area and in cubbies in stars classroom and the red room classroom. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 1/7/2026. Priority foundation violation 7-38-020(a) citation issued.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed improper temperature of 104f of the wash solution in low temperature dish machine. Instructed to repair low temperature dish machine the wash solution in spray type warewashers that use chemicals to sanitize may not be less than 120f. (note at time of inspection low temperature dish machine was not in use. ) priority foundation violation 7-38-025 no citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed no water pressure at right handwashing sink in boys washroom. Instructed to restore water pressure to right handwashing sink in boys washroom and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed door of staff washroom in lions classroom, in rear dry storage area and red room classroom to not be self closing. Instructed to install self closing device on staff washroom doors in lions classroom, rear dry storage area and red room classroom so that staff washroom doors shall be tight fitting and self closing.
55. Physical facilities installed, maintained & clean
- comments: observed unused ice machine in rear dry storage. Instructed to remove items that are unnecessary to the operation or maintenance of the establishment that is nonfunctional or no longer used.
