- comments: observed food items stored on floor in prep, storage, and dish washing areas. Instructed manager to elevate all food items six inches off floor.

- comments: observed single service utensils not inverted. Instructed manager to invert all single service articles to protect from contamination.
55. Physical facilities installed, maintained & clean
- comments: observed dust buildup on ceiling vents and light shields in prep area, debris on floors under all equipment, along walls and in all corners in prep, storage, dish washing and office areas. Instructed maanger to clean and maintain.

- comments: observed damaged light shield in prep area. Instructed manager to replace and maintain.

- comments: previous core violations not corrected from report #2580802 on 8-28-23:39- must provide a splash guard in between 3- compartment sink and 1- compartment. 47-must repair or replace worn door gasket on walk in coolermust remove rust from lower shelf at prep table. 58-observed no food allergen training for food managers must provide and maintain.
