- comments: observed blank copies of employee health policy on site. Instructed facility to maintain policy with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle bodily fluid spills on site. Priority foundation violation #7-38-005. Citation issued.

- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at hand sink (90. 1f) instructed person in charge hot water at all hand washing sinks must be at least 100f. Plumber on site and corrected handsink now reading 102. 4f priority violation #7-38-030(c). Citation issued.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed the interior of the reach in coolers in front and rear prep in need of cleaning. Instructed to detail clean.

- comments: observed 3-compartment sink at bar pipes completely fallen off, instructed to repair and maintain plumber is on site.

- comments: observed missing and damaged floor tiles in the following areas: men & womens toilet room, thoughout prep areas, all walk-in coolers & walk-in freezer, bakery prep, bakery walk-in cooler, front and rear prep. Noted floor grout missing throughout the areas mentioned above. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed chipped paint on the walls in the front and rear prep, ceiling in pizza prep, rear prep, and dish area. Instructed to resurface and repaint chipped areas to ensure walls are sealed, smooth and easily cleanable.

- comments: noted continued non-compliance from report #2597793 conducted on 7/19/24. Previous core violations not corrected: (39)3-305. 11- observed disposable paper towel dispenser for main prep line handwashing sink directly above uncovered, exposed food in line coolers, posing risk of contamination from splash or drip after handwashing. Instructed to store food covered in this area or relocate paper towel dispenser from above exposed food item to eliminate risk of contamination from splash or drip and maintain. (47) 4-101. 19 - observed dry, heavily-soiled wiping cloths used to line nonfood-contact surfaces of cold-holding equipment (used to line edges of compressor access doors). Instructed to remove soiled wiping cloths and replace with a material that is smooth and easily-cleanable for nonfood-contact surfaces and maintain. Priority foundation violation #7-42-090. Citation issued.
