- comments: observed no city of chicago certified food manager on site upon arrival while tcs foods were prepared. Instructed manager to ensure city of chicago certified food manager is on site at all times. Priority foundation violation 7-38-012 citation issued.

- comments: observed 2 large flies sitting directly on top of beef in the walk-in cooler. Beef stored in open pan within walk-in cooler. Instructed manager to ensure food is protected from contamination of pests. The manager discarded 8 pounds of beef ($55), and a priority violation 7-38-005 citation was issued.

- comments: observed dish machine in bar area with a chlorine sanitizer concentration of 0 ppm. Instructed manager to ensure chlorine sanitizer solution reaches a minimum concentration of 50 ppm. Employee utilizing 3-compartment sink at time of inspection. Dish machine tagged. Priority violation 7-38-025 no citation issued.

- comments: observed 8 large flies throughout the facility, including in the basement dish/ prep area, in the walk-in cooler, in the main lobby, and the main floor kitchen. Instructed manager to minimize fly activity. Priority foundation violation 7-38-020(a) citation issued.
