- comments: observed ice/food not protected. Interior of the ice machines: ice bin and all ice contact sides with black slime substance dripping down onto the ice also, we observed extremely rusty screw at metal panel of inside the ice bin. Leaking into ice cubes. Instructed to clean and sanitize the interior of the ice machine throughout, also replace mentioned screws. Priority foundation violation 7-38-005 citation issued.

- comments: replace broken wraps on the vent duct in the basement storage area.
55. Physical facilities installed, maintained & clean
- comments: broken, missing ceiling above the ice machine, shelves, and soda boxes in storage basement area. Also, repair walls behind the soda boxes in the mentioned area. Broken cement floor with standing water. In front of the ice machine must be repaired.
