- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: noted: facility did not provide a valid city of chicago food service sanitation certificate for employees working on site. Informed a certified manager with a current city of chicago certificate must be on premises at all times and a valid must remain posted on site whenever time and temperature control for safety(tcs) foods are being prepared, handled, and/or served. Priority foundation violation #7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no adequate supplies and no policy on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Instructed to provide adequate supplies for vomit and diarrhea cleanup. Noted: facility went out and purchased a cleanup kit. However, did not provide a cleanup policy. Instructed to provide. Priority foundation violation #7-38-005. Citation issued.

- comments: observed handsink in basement prep area and barn is not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handler obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.

- comments: observed no paper towel at hand-sink in the bar and basement prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation combined with the above violation

- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at men's washroom handsink (68. 7�f). Instructed person in charge of hot water at all hand washing sinks must be at least 100�f. Priority violation #7-38-030(c). Citation issued.

- comments: observed ice machine located in basement with a build-up of black slime substance, dripping onto ice cubes stored below the ice bin. Ice is used for human consumption. Also black slime at the unit's exterior between the ice bin and the top part of the unit. Interior plate of ice bin with excess rust; instructed to clean, sanitize, and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: noted low temperature dish washing machine in use at bar and rear prep is not properly sanitizing. Sanitizing strength of sanitizer (chlorine) at the time of inspection was 0 ppm. Instructed not to use unit until reinspected by chicago department of health (cdph). Tagged unit. Priority violation #7-38-025. Citation issued.

- comments: noted internal temperatures of the following time/temperature control for safety food items inside the cold holding unit to be improper: raw shelled eggs ranging in temp of 48. 6f-56. 8f, cooked potatoes at 48. 6f. Manager immediately and voluntarily disposed of 15 lbs of products worth $250. 00 through the denaturing process. Priority violation #7-38-005. Citation issued.

- comments: observed no disclosure/reminder for consumer advisory on menu presented for all animal food products that can be ordered raw orundercooked(ex:sunny side eggs and poached eggs on brunch menu). Facility has use of electronic menus, instructed to provide consumer advisory on all menus. Priority foundation 7-38-005

- comments: observed facility not meeting compliance with variance/specialized process; noted in facility (rop) reduce oxygen packaging of cooked meat with no date. In prep kitchen walk in cooler. Informed person in charge of requirements and facility must meet and maintain. Tag unit and food products. Priority violation 7-38-005 citation issued.
32. Variance obtained for specialized processing methods
- comments: noted a vacuum packaging machine on site for food items inside the walk-in cooler vacuum packaged with no proof of haccp or variance per health code requirement. Instructed management to discontinue the vacuum packaging process until a variance and haccp are approved by the health department. The food items that were vacuum packaged were also tagged and held for inspection. Cooked pork, about 24 lbs, value of $250. 00 priority foundation violation #7-38-005. Citation consolidated with #29.

- comments: observed no ambient air thermometers inside the reach in coolers and walk-in coolers. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to monitor the ambient air temperature of equipment properly.

- comments: observed food containers stored on the floor in the walk-in cooler, prep, and storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed three compartment sink in dishwashing area not draining properly (slow draining), water released from one compartment (basin) backup inside another compartment (basin) when released inside sink. Informed person in charge of requirements and must repair and maintain the sink. Plumbers on site, 3 comp sinks no longer backing up from one basin to another basin. Priority violation 7-38-030(c) citation issued.

- comments: noted continued non-compliance from report #2582462 conducted on 9/27/23. Previous core violations not corrected: (#47)4-101. 19 observed excessive rust buildup on the bottom shelves of two stainless steel prep tables in the food prep area. Management instructed to replace the tables or paint the tables to provide a smooth and cleanable surface. (#47) 4-501. 11 repair the bottom drawer of the 3 door prep cooler located at the end of the cook's line, so it closes properly. (#49) 4-602. 13 clean the interior of the deep fryers, the top of the dish machine, and the grill. Clean the dust buildup on the top shelves of the prep tables above the prep coolers.
