- comments: 3-603. 11 observed no disclosure/reminder for consumer advisory on menu presented for all animal food products that can be ordered raw orundercooked (ex:sunny side eggs and poached eggs on brunch menu). Facility has use of electronic menus, instructed to provide consumer advisory on all menus. Priority foundation 7-38-005

- comments: 4-301. 12 observed 3-compartment sink stoppers missing at bar and dish area. Instructed manager must provide working stoppers at 3-compartment sink.

Management instructed to replace the tables or paint the tables to provide a smooth and cleanable surface. (#47) 4-501. 11 repair the bottom drawer of the 3 door prep cooler located at the end of the cook's line, so it closes properly. (#49) 4-602. 13 clean the interior of the deep fryers, the top of the dish machine, and the grill. Clean the dust buildup on the top shelves of the prep tables above the prep coolers.
