Report date: May 1, 20258. Hands clean & properly washed
- comments: observed employees not washing hands when changing tasks. Observed food employees handle raw meat and then prepare food orders and take orders by touching the register and then preparing food items without washing hands. Instructed to always wash hands after engaging in activities that contaminate the hands. Employees washed their hands when notified. Priority violation 7-38-010, citation issued.
22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods. Found rib (82. 4f) and gyro (74. 3f) premade egg rolls out at room temperature in the rear prep area. Instructed to keep tcs food in cold holding units to maintain a cold holding temperature of 41. 0�f or below, until food items are ready to be cooked. Manager discarded approximately 20 lbs of product worth $90. Must have cold holding foods at 41. 0�f or below. Priority 7-38-005, citation issued
35. Approved thawing methods used
- comments: observed improper thawing methods for tcs foods. Observed chicken in a container of still water. Also, found raw, packaged beef sitting on the drainboard of the warewashing sink. Instructed to use proper thawing methods and maintain.
38. Insects, rodents, & animals not present
- comments: observed approximately 60 or more rat droppings on site. Found droppings in the following areas on the floor: in the crevices of the wall bases near the rear exit door and front service area, under and behind the cooking equipment under the ventilation hood and the warewashing sink, surrounding all the cold holding units in the front service area. Aslo, found rat droppings on top of a can of nacho cheese on a shelf under a prep table in the rear prep area. Instructed to remove droppings, clean, and sanitize all affected areas. Priority foundation violation. 7-38-020(a), citation issued.

- comments: observed a large rodent sticky trap on the shelf under the food prep table next to the microwave in the rear food prep area. Instructed to remove sticky trap, only use products from pest control company, and refrain from storing on mentioned surfaces.

- comments: observed the hand soap dispenser in poor condition in the rear food prep/ dishwashing area. Also, found a bowl of soap with a spoon being used as a soap dispenser. Instructed to provide an adequate soap dispenser and maintain.

- comments: observed dried flour, paste residue on the top of the three door cold holding unit across from warewashing sink in the rear prep area. Instructed to clean more frequently and maintain.

- comments: observed no food handler certificates on site at the time of the inspection. Instructed all employees who are not certified food managers to take food handler course and maintain records on site at all times.

- comments: observed no allergen certificate on site for certified food manager. Instructed all certified food managers must take course and maintain records on site.
