Report date: Jan 24, 202522. Proper cold holding temperatures
- comments: observed improper cold-holding temperatures of tcs foods inside the basement walk-in cooler. Observed milk at 46f, eggs at 46f, cut lettuce at 47f, sour cream at 45f, garlic tahini dressing at 45f, macaroni and cheese at 45f, whipping cream at 45f, cheese at 46f, cream cheese at 47f, and 1000 island dressing at 44f. Management instructed to maintain all cold foods below 41f. Priority violation 7-38-005. Citation issued.
23. Proper date marking and disposition
- comments: observed tcs foods (caesar dressing and garlic tahini dressing) stored in the walk-in cooler and refrigerators beyond the 7-day maximum use-by date. Management instructed to date mark all tcs foods stored more than 24 hours with the expiration date not to exceed 7 days and including the preparation date as day #1. Priority violation 7-38-005. Citation issued.
29. Compliance with variance/specialized process/haccp
- comments: observed pickled carrots and other vegetables stored in the walk-in cooler that are pickled onsite without an approved haccp plan. Management instructed that an approved haccp plan by cdph is needed to pickle foods on the premises. Priority foundation violation 7-38-005. Citation issued.
32. Variance obtained for specialized processing methods
- comments: observed no cdph-approved haccp plan on the premises for pickled carrots and vegetables stored in the walk-in cooler. Management instructed that an approved haccp plan is needed to pickle foods on the premises. Priority foundation violation 7-38-005. See violation #29 above for citation.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the basement walk-in cooler next to the dishwashing and food prep area with an air temperature of 46f. The cooler was used to store tcs foods such as cooked foods, sauces and cheeses. Management instructed to repair and maintain the cooler below 41f. Priority violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed a pest infestation of live cockroaches on the premises. Observed over 100 live juvenile and adult cockroaches on the floor, walls, ceiling, around and inside of the water heater and chest freezer, in the dump sink next to the dishwashing machine, on food contact equipment such as cutting boards, and in storage containers in the basement food prep and dishwashing area. Management instructed to eliminate the pest activity throughout the premises. Priority violation 7-38-020(a). Citation issued.
49. Non-food/food contact surfaces clean
- comments: clean the interior of the deep fryers with grease deposits inside of them.
51. Plumbing installed; proper backflow devices
- comments: observed the 2nd floor exposed hand washing sink behind the bar to be slow draining. Management instructed to repair the hand washing sink so that it drains properly.

- comments: clean the grease on the walls of the food prep area, especially the back wall between the prep cooler and deep fryers.

- comments: observed chipped and bubbling paint on the ceiling of the basement dishwashing and food prep area and above the exposed hand-washing sink in the smoke house prep area. Management instructed to scrape and repaint the affected areas. Observed the floor in front of the cook's line with gouges and crevices. Management instructed that the floor must be repaired and must be smooth and easily cleanable. Observed gaps between the stainless steel walls in the basement dishwashing area. Management instructed to seal all openings to prevent pest harborage conditions.
