- comments: observed no city of chicago certified food manager on site with valid original certificate posted fpr public view ehile preparing and serving tcs foods (mac and cheese, greens, chicken, tilapia. Instructed to procide at least one on site at all times while open and operating. Priority foundation violation. #7-38-012. Citation issued.

- comments: observed missing thermometers inside cooling unit with cold hold attached in prep area. Instructed to provide.

- comments: observed no pest control logbook or proof of service provided on site during time of inspection. Instructed to provide and maintain on site for review as required. Priority foundation violation. #7-38-020 (c). Citation issued.

- comments: observed food handler without hair restraint and preparing food. Instructed to provide.

- comments: observed pipes attached to walls in prep area behind deep fryers, fire estinguisher and, and along side prep table used for fish prep and cooling unit with cold hold attached with grease and food debris. Instructed to remove grease buildup regularly and in order to prevent pest harborage.

- comments: observed one compartment sink located in dishwashing area with heavy leak on piping causing water to pool on the floor. Instructed to repair and maintain in good condition.

- comments: observed plubming fixtures in both toilet rooms in need of cleaning. Instructed to provide and maintain.

- comments: observed outside refuse area with excessive amount of trash and unknown debris around receptacles. Instructed to remove and maintain area.

- comments: observed floors throughout seating area with food debris throughout booth spaces, heavy accumulated grease and food debris under fryers and throughout prep area inder prep shelves, bulk rolling carts, steam tables etc. Instructed to clean and maintain.
