- comments: observed no certified manager on duty between 1:50-2:20pm while tcs foods such as chicken were prepared. Instructed to maintain employee holding city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed mussels on site and shellstock tags not maintained on file. Management voluntarily discarded and denatured approximately 3lbs of mussesl valued at approximately $100. Instructed to maintain shellstock tags on site as required by law. Priority foundation violation 7-38-005. Citation issued.
