- comments: 2-101. 11 observed the person in charge without a city of chicago food service sanitation manager's certificate on site and posted. Instructed all persons in charge must obtain a city of chicago food service sanitation manager's certificate and have said original/valid certificate posted at all times. Priority foundation. Citation issued under violation 2.

- comments: 2-101. 11 observed no original/valid city of chicago food service sanitation manager's certificate posted. Instructed to always post an original/valid city of chicago food service sanitation manager's certificate at all times. Priority foundation. Citation issued 7-38-012.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 10 rodent droppings located in the corner by the grease trap under the 3 compartment sink and about 15 rodent droppings located in the furnance room on the storage shelves. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 15 observed a 1/4 inch gap at the bottom of the rear door located by the toilet rooms. Instructed to make said door tight fitting and rodent proofed.
39. Contamination prevented during food preparation, storage & display
- comments: 3-305. 14 observed about 2 outdoor use grills and about 6 outdoor use smokers stored outside in the rear of the facility. Observed 2 bags of outdoor use charcoal stored inside of the facility by the rear door. Instructed to remove all outdoor use grills, smokers and charcoal. Outdoor cooking is prohibited by the city of chicago food code rules and regulations. At the time of the inspection there was no outdoor cooking of foods taking place. Priority foundation. No citation issued.

- comments: 4-502. 13 observed cardboard being reused as a shelf liner in the food prep area. Instructed to remove cardboard, all surfaces should be smooth and easily cleanable.

- comments: 4-601. 11(c) observed a blood spill inside of the freezer. Instructed to clean and sanitize.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 observed no covered receptacle inside of the women's toilet room. Instructed to provide.

- comments: 6-201. 13 observed coving deattached and missing in various areas of the food prep area. Instructed to replace coving and make coving tight fitting throughout the food prep area.

- comments: 6-501. 114 observed the furnance room and rear area outside of the furnance room with unnecessary clutter and items stored on the ground. Instructed to organize, remove unnecessary clutter and elevate everything at least 6 inches from the ground.
